PRIVACY NOTICE for GENERAL DATA PROTECTION REGULATIONS
In line with the new GENERAL DATA PROTECTION REGULATIONS MAY 2018, please find information below about my compliance with the new regulations regarding protection of your stored data.
I have a record of what personal data I hold, and what it is used for. Patients know I have their personal data and understand how I use it. I only collect the personal data I need to carry out my professional work, and to keep it accurate and up to date and secure for as long as it is needed. You may exercise your rights regarding the personal data I hold about you
How I collect information from you
I obtain information about you when you phone to book an appointment with me; or my online secretarial service;
if you make an email enquiry; or online via my practice website, www.dralicegreene.com. (This site is itself GDPR compliant).
In face to face consultation, I collect and retain information about you which is for the purposes of professional treatment or therapy work which we have agreed together beforehand.
Type of information I collect from you
The personal information I collect might include your name, address, email address, telephone numbers etc. including personal information necessary to carry out our agreed professional work. All files/notes containing personal information are kept securely locked. Electronic information is password protected.
How I use your information
We may use your information to: contact you while you are undergoing therapy/treatment with us; to confirm session bookings; to respond to your requests if you contact us with a query; to manage the practice accounts; to notify you of changes to appointments; to send you communications pertinent to you, mostly sending agreed documents, or information regarding your queries and appointments.
Document Retention and Data Deletion
I review the retention periods for personal information on a regular basis. I am legally required to hold some types of information to fulfil my statutory obligations. Your personal medical/treatment notes are held in either paper or digital form only, as completely confidential, in a locked and secure password protected environment. Your personal treatment notes may be stored for at least 7 years in compliance with my professional indemnity requirements. After that time, they will be confidentially shredded by a professional waste operator.
Who has access to your information?
If you have contacted the Practice by email through its website www.dralicegreene.com, your email information only will be retained for the duration of treatment, in a password protected computer programme to which only Dr Greene has password access. Your email information or contact form will not be stored by my website provider, and will not be shared with third parties, unless requested by you for confidential medical purposes only.
Third Party Service Providers
I may sometimes use a third party online Appointments provider to book appointments, and disclose to them only the personal information that is necessary to deliver the service. I have a contract in place that requires them to keep your information secure and not to use it for their own direct marketing purposes. Please be reassured that I will not release your information to third parties beyond the service required unless you have requested me to do so, or I am required to do so by law, for example, by a court order or for the purposes of prevention of fraud or other crime.
Your choices
I will not contact you for marketing purposes by mail, email, phone or text message unless you have given your prior consent.
How you can update your information
The accuracy of your information is important to me. I am happy to review and correct any information that I hold about you.

How you can access your information if need be.
You have the right to ask for a copy of the information I hold about you. If you exercise your rights under data protection law,
I will respond no later than one calendar month, starting from the day after I receive your request.
